If you are not an US citizen you should use the W-8 Form
Choose Entity Type: Foreign Individual

leave the Taxpayer ID Number blank.
If you get problems with your Taxpayer ID Number then enter some digits.

The payment requirement is, you must be subscribed step 3 for 45 days prior to payday!.. Then only you are eligible for Payments! and thank you for watching video!
